ユタ日本語補習校（以下「本校」）は、児童生徒および保護者の個人情報を、以下のとおり取り扱います。

1. 取得する情報

  • 児童生徒: 氏名、生年月日、学年、在籍状況、健康に関する情報（アレルギー・既往歴など）
  • 保護者: 氏名、住所、電話番号（携帯電話番号を含む）、メールアドレス、緊急連絡先

2. 利用目的

  • 授業・行事の運営、名簿および成績の管理
  • 保護者への連絡（入学手続き、年度更新、成績、図書、保健室からの連絡）
  • 緊急時の連絡（避難、臨時休校、引き渡しなど）
  • 児童生徒の安全確保（健康情報は保健室および担任が確認します）

3. テキストメッセージ（SMS）による緊急連絡

保護者および教職員の同意をいただいた場合にのみ、緊急連絡をテキストメッセージでお送りします。

  • 送信元: ユタ日本語補習校（UJLS）
  • 送る内容: 緊急連絡のみ（避難、臨時休校、引き渡し）。宣伝は送りません
  • 頻度: 年に数回
  • 通信料: お客様の通信事業者の料金が適用されます
  • 停止: STOP と返信すると停止します。HELP で問い合わせ先を返します

同意は本校のポータル（ログインが必要）の「アカウント → 通知」からいつでも変更できます。同意した日時と、同意時に表示した文面を記録します。

携帯電話番号および SMS の同意情報は、いかなる第三者・関連団体にも提供・共有・販売しません。 宣伝や勧誘の目的で使用することもありません。

4. 第三者への提供

本校は、次の場合を除き、個人情報を第三者に提供しません。

  • ご本人の同意がある場合
  • 法令に基づく場合
  • 児童生徒の生命・身体の安全のために緊急に必要な場合

システムの運用のため、次の事業者のサービスを利用しています。いずれも本校の指示の範囲でのみ情報を取り扱います。

  • Amazon Web Services（サーバー、メール送信、テキストメッセージ送信）
  • Google Workspace（メール、連絡先）

5. 保管期間

在籍中および卒業・退学後も、在籍および成績の記録は証明書の発行に備えて保管します。卒業・退学された方の情報は「卒業生」として残り、在校生向けの連絡の対象からは外れます。

携帯電話番号と SMS の同意情報は例外です。 同意を解除された時点で、携帯電話番号と同意の記録は削除します。

6. 開示・訂正・削除のご請求

ご本人またはその保護者からのご請求により、個人情報の開示・訂正・削除に応じます。下記までご連絡ください。

Privacy Policy

Utah Japanese Language School (“UJLS”, “we”) handles the personal information of students and their parents/guardians as described below.

1. Information we collect

  • Students: name, date of birth, grade, enrollment status, health information (allergies, medical history)
  • Parents/Guardians: name, address, phone number (including mobile number), email address, emergency contacts

2. How we use it

  • Operating classes and school events; maintaining rosters and grade records
  • Contacting parents (admissions, annual updates, grades, library, health office)
  • Emergency notifications (evacuation, school closure, student pick-up)
  • Keeping students safe (health information is reviewed by the health office and the homeroom teacher)

3. Emergency notifications by text message (SMS)

We send emergency text messages only to parents and staff who have opted in.

  • Sender: Utah Japanese Language School (UJLS)
  • Content: emergency notifications only (evacuation, school closure, student pick-up). We do not send marketing messages
  • Frequency: a few times per year
  • Message and data rates may apply
  • To stop: reply STOP. Reply HELP for help

Parents and staff can change this at any time in our school portal (login required) under Account → Notifications. We record the date and time of consent and the exact consent text shown at that time.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.

4. Sharing with third parties

We do not share personal information with third parties except:

  • with your consent
  • as required by law
  • when urgently necessary to protect a student’s life or safety

We use the following service providers to operate our systems. They process information only as instructed by us.

  • Amazon Web Services (hosting, email delivery, text message delivery)
  • Google Workspace (email, contacts)

5. Retention

We retain enrollment and academic records while a student is enrolled and after graduation or withdrawal, so that we can issue certificates. Records of former students are kept as alumni records and are removed from messages sent to currently enrolled families.

Mobile numbers and SMS consent are an exception. When you opt out, we delete your mobile number and the record of your consent.

6. Access, correction and deletion

Upon request by the individual or their parent/guardian, we will provide access to, correct, or delete personal information. Please contact us below.
